What you'll do

• Assist patients with bathing, grooming, dressing, toileting, and personal hygiene
• assist with mobility, ambulation, transfers, and directed exercises
• take and document vital signs as appropriate
• assist with meal preparation and daily living activities
• follow established plans of care
• observe and report changes
• collaborate with nurses, therapists, physicians, and families
• complete timely visit documentation
• provide companionship and emotional support.
