What you'll do

• Complete initial and ongoing assessments, including OASIS
• reevaluate nursing needs and outcomes
• develop and revise plans of care
• provide nursing procedures and plan discharges
• communicate with clients, families, providers, and team members
• coordinate services and report changes
• monitor payer compliance
• document care
• supervise, teach, and direct nursing personnel
• perform assigned on-call responsibilities.

Why this role
