The Registered Nurse (RN) Case Manager role is a hybrid field nurse and manager position serving group homes in Lorain County, OH, with visits Monday through Friday evenings and one weekend assignment per month. The case manager oversees long-term care by assessing needs, planning and coordinating home-care services, and delivering nursing care based on individual plans. The role also serves as a liaison among patients, families, caregivers, and clinical and administrative staff, while participating in an on-call rotation.

What you'll do

• Assess patients and home-care needs
• complete physical assessments and histories
• assist patients and caregivers with care planning
• provide preventive and rehabilitative nursing procedures
• administer prescribed medications and treatments
• communicate with clinical staff and treatment teams
• participate in care conferences
• arrange necessary equipment
• supervise and evaluate LPN services as assigned
• perform OASIS assessments
• participate in scheduled on-call rotation.

Why this role

Requires active, unencumbered Ohio RN licensure, at least one year of RN experience, CPR certification, a valid driver’s license, and auto insurance. Schedule includes Monday–Friday, 4:45 PM–9:00 PM, one weekend assignment per month, and on-call coverage for the assigned territory and company-wide every nine weeks.
