What you'll do

• Provide direct care according to individualized plans
• assist with daily living, hygiene, and medical needs
• lead and train direct support professionals
• coordinate daily activities and implement therapeutic and behavior supports
• collaborate on care plans
• document progress and incidents
• monitor health and behavior
• maintain regulatory compliance and accurate records
• communicate with families and advocates
• maintain a safe, clean home and follow emergency and medication protocols.
