Registered Nurse (RN) Case Manager — Home Care/Hospice

The Registered Nurse (RN) Case Manager — Home Care/Hospice role is onsite in Onamia, MN. The schedule is Monday through Friday, eight-hour day shifts, with rotating weekend and holiday call. Pay is $38.19–$57.29 per hour. The role supports Mille Lacs Health System’s Home Care and Hospice services. The case manager implements and facilitates client plans of care, coordinates with other disciplines, and provides direct oversight of client care. Assessments, advocacy, referrals, and safety monitoring help coordinate appropriate services for clients and their care teams.

What you'll do

• Complete new-client admission assessments within five days
• maintain clear and accurate charting
• advocate for clients and refer them to appropriate resources
• coordinate care among multiple and outside care teams
• assess and evaluate clients
• maintain five visits and documentation in an eight-hour day unless approved otherwise
• follow infection-control practices
• report safety concerns and take measures to promote safety
• support cost-effective services and monitor supply use
• oversee licensed and non-licensed Home Care and Hospice personnel.
