What you'll do

• Provide skilled nursing care in home and school settings
• assess and monitor patient condition and communicate changes to the care team
• administer medications, treatments, and prescribed therapies
• provide tracheostomy, ventilator, airway, and G-tube care as needed
• maintain documentation
• educate and support families
• follow individualized care plans and maintain a safe care environment.
