What you'll do

• Investigate reports involving adults aged 60 or older and people with disabilities aged 18–59
• assess safety and basic care needs and support the least restrictive environment
• complete Illinois Department on Aging certification training
• maintain a caseload with timely documentation and care planning
• collaborate with community resources
• respect client self-determination and confidentiality
• conduct local visits and maintain case notes and reports.

Why this role
