What you'll do

• Develop outreach strategies
• maintain relationships with healthcare, behavioral health, social service, child welfare, public health, and recovery partners
• conduct presentations and community engagement
• represent the program at meetings and events
• develop targeted rural outreach
• maintain records of outreach, referrals, admissions, waiting lists, and partnerships
• track access metrics
• prepare required reports
• document activities and outcomes in electronic systems
• gather referral-source feedback
• and implement quarterly referral-management action plans.
