Independent Community Support Worker

What you'll do

• Support daily routines, organization, and independent living skills
• help individuals practice meal planning, shopping, budgeting, and household tasks
• encourage participation in community activities, appointments, and social opportunities
• promote choice, self-advocacy, and independence
• follow individualized service plans and document services, progress, and concerns
• communicate with the supervisor and care team
• assist with personal care only when included in the service plan and within training.
