What you'll do

• Plan, organize, implement, evaluate, and direct social services

• interview patients, residents, and families
• provide information about Medicare, Medicaid, and financial assistance
• protect confidential health information
• contribute to social treatment care plans
• coordinate facility tours, admission paperwork, and clinical and financial approvals.
