Registered Nurse Case Manager — Home Care

What you'll do

• Coordinate patient care plans across settings
• assess patients and develop or adjust care plans
• provide health instructions
• monitor progress and outcomes
• educate patients and families
• conduct supervisory visits every 90 days, or every 60 days if CAP
• communicate changes in client condition to the primary nurse and aide concerns to the field supervisor
• reevaluate nursing needs.

Why this role
