The Community Health Worker (CHW) – Certified Nursing Assistant (CNA) role is onsite, serving patients in the Kingstree, Effingham, and Pamplico area of South Carolina. Compensation is not specified. Your Health Organization provides primary and specialty care, pharmacy support, and coordinated services to patients in homes, facilities, clinics, and virtual visits.

What you'll do

• Coordinate patient care with care-team members and facilitate provider, therapy, and telehealth visits in homes and assisted living facilities. Deliver medications and durable medical equipment, review medication instructions, and reconcile medication lists. Train and support patients using remote patient monitoring equipment
• assemble and explain DME
• collect blood specimens and perform routine venipuncture as needed. Daily travel between homes and facilities is required.
