What you'll do

• Assess patients by reviewing medical history, medications, and anticoagulation-related laboratory results
• monitor and adjust Coumadin dosages according to established protocols and physician guidance to maintain therapeutic INR levels
• educate patients and families about therapy, side effects, dietary considerations, and adherence
• collaborate with interdisciplinary teams on individualized care plans and continuity of care
• document interactions, dosage adjustments, and findings in electronic health records
• respond to adverse reactions or complications
• participate in quality improvement and follow clinical guidelines and regulatory standards.

Why this role

Requires a current, valid U.S. Registered Nurse license, at least two years of clinical nursing experience, BLS certification, knowledge of anticoagulants—particularly warfarin—and EHR proficiency. Anticoagulation certification, outpatient clinic experience, ACLS, motivational interviewing, and quality-improvement experience are preferred.
