What you'll do

• Visit and counsel clients and families to identify service needs and provide assistance or referrals
• assist clinicians with crisis intervention
• participate in case conferences and treatment-team meetings
• prepare progress notes and summaries
• accompany clients to appointments
• coordinate with schools and other organizations
• facilitate groups
• maintain a safe environment. May provide Intensive Care Coordination as the agency’s appointed coordinator, including supporting treatment planning and monitoring progress.
