What you'll do

• Monitor daily vital signs and biometric data through the remote monitoring platform
• review blood pressure, pulse, oxygen saturation, weight, blood glucose, temperature, and other data
• identify abnormal trends and escalate significant findings
• call patients to assess symptoms, provide education, and encourage care-plan adherence
• document interactions and interventions in the electronic medical record
• collaborate with providers and the clinical team
• and meet Medicare RPM documentation requirements.
