The Registered Nurse (RN) – Home Health – Part Time role is field-based, providing care in patients’ homes within an assigned territory in Bryan, TX. Compensation is not specified. VitalCaring’s Home Health team supports recovery, chronic-condition management, and patient independence at home. As the clinical lead for assigned patients, the RN assesses needs, manages individualized care plans, delivers skilled nursing care, and coordinates with physicians, therapists, and aides. Patient education, progress monitoring, and discharge coordination contribute to recovery and help reduce hospital readmissions. The role participates in an on-call rotation as appropriate.

What you'll do

• Conduct comprehensive in-home assessments
• develop and manage individualized care plans with physicians and care teams
• provide skilled nursing, medication management, treatments, and interventions
• coordinate care across therapists, aides, and physicians
• monitor progress and make clinical decisions
• educate patients and families
• coordinate transitions and discharge planning
• maintain timely EMR documentation
• supervise and support Home Health Aides as needed
• participate in on-call rotation as appropriate.
