What you'll do

• Develop and expand referral relationships with physicians, hospitals, senior living communities, rehabilitation centers, and skilled nursing facilities
• identify opportunities to grow admissions
• build partnerships with physicians, case managers, social workers, and healthcare leaders
• support patients and families through care decisions and hospice transitions
• develop and execute a territory plan
• prioritize relationships and adapt the approach to support growth and quality of care.
