What you'll do

• Handles intake calls from potential transplant candidates
• assesses patients for waiting list placement
• coordinates evaluations with the transplant team and referring physicians
• facilitates pre- and post-transplant management
• communicates with UNOS to maintain accurate waiting lists
• educates patients and families on nutrition and diagnostic procedures
• maintains patient records and charts.
