Medicare MDS Coordinator

Job overview

The Medicare MDS Coordinator role is onsite in Eureka, CA, with pay of $30–$33 per hour. Granada Rehabilitation & Wellness Center provides resident care in a rehabilitation and wellness setting. Under the Director of Nursing Services, the coordinator organizes the interdisciplinary team’s MDS assessment process in accordance with state and federal regulations. The role supports timely assessments, coordinated care planning, nursing interventions as needed, and accurate documentation to promote quality resident care and protect residents’ rights.

What you'll do

• Coordinate the Medicare/MDS resident assessment process and ensure the interdisciplinary team completes assessments on time
• coordinate development, implementation, and evaluation of care plans
• provide or coordinate nursing treatments, medications, and interventions as indicated by the care plan or physician orders
• coordinate nursing care in accordance with infection-control standards
• follow safety policies and initiate emergency measures as needed
• ensure essential information is exchanged and documentation meets regulatory and company requirements
• participate in assigned meetings and in-services
• protect residents’ rights and address consumer concerns according to company standards.

Why this role

Requires current nursing licensure as an LVN or RN. CPR certification and two years of long-term-care nursing experience are preferred; supervisory experience is preferred.
