What you'll do

• Provide crisis intervention
• assess strengths, needs, abilities, and preferences
• develop and carry out housing stability plans
• monitor progress and document services
• enforce program rules and regulatory requirements
• connect clients with community resources
• facilitate moves to transitional or permanent housing when appropriate
• conduct home visits when appropriate
• determine eligibility
• process temporary financial assistance
• maintain case records in HSMIS.
