What you'll do
- Prepare intake evaluations and diagnostic assessments
- assess mental health needs, risk factors, and housing barriers
- provide individual, group, and family therapy using trauma-informed care
- conduct psycho-educational groups
- collaborate with clinical staff, case managers, housing specialists, and psychiatrists
- maintain clinical records in Millennium
- conduct outreach and follow-up
- assist clients with housing-system navigation and referrals
- provide community-resource referrals
- participate in supervision, multidisciplinary meetings, and quality improvement.
