What you'll do

• Develop and maintain referral relationships with healthcare and community partners
• conduct field prospecting, account visits, education, presentations, and follow-up
• identify territory growth opportunities
• explain hospice philosophy, palliative care, eligibility basics, and referral processes

• track outreach and referral outcomes
• attend community events.
