The RN guides high-risk patients with chronic conditions through self-management and care planning and manages cases during high-risk transitions. The role coordinates services and clinical resources to support continuity, informed decisions, and quality care.

What you'll do

• Provide case management and utilization review and planning
• coordinate social services within patient care
• follow population health and chronic disease management protocols
• ensure quality care and regulatory compliance
• collaborate with clinical staff on care plans and goals
• support informed decisions by patients and families
• ensure tests and labs are appropriate and completed on time
• promote efficient use of clinical resources.
