What you'll do

• Provide personal care and assist with activities of daily living according to the plan of care
• support safe ambulation and transfers
• assist with exercises and nutritional snacks as directed
• report changes in patient condition
• document care and observations
• complete charting, timesheets, and mileage logs
• attend patient care conferences, team meetings, and in-services
• participate in the on-call rotation when scheduled for Home Care.
