Case Manager — Boston

Job overview

What you'll do

• Conduct weekly, bi-weekly, or monthly client sessions depending on service needs
• communicate with clients, mentors or companions, family members, clinical and non-clinical providers, and financial guarantors
• develop and revise reintegration plans
• provide phone support
• collaborate with and support mentors or companions
• pre-approve related out-of-pocket reimbursements
• document team interactions
• manage toxicology-screen information and medication management in conjunction with mentors or companions
• hold a valid driver’s license and transport clients if needed.
