What you'll do

• Assess patients’ and caregivers’ biomedical, psychological, social, and functional needs
• develop patient-centered care plans
• engage patients and families through advocacy, communication, education, and resource facilitation
• coordinate transitions between care settings
• collaborate with treatment teams and external providers
• evaluate and adjust care plans
• facilitate case conferences and process-improvement activities
• support community health workers and post-acute care providers.
