What you'll do
- Complete hospice admission visits in homes, hospitals, facilities, and other care settings
- perform comprehensive nursing assessments
- educate patients and families about hospice services, medications, equipment, and expectations
- complete admission documentation and consents
- collaborate with physicians and the interdisciplinary team
- coordinate medications, durable medical equipment, supplies, and immediate needs
- communicate patient information to the RN Case Manager and clinical team
- document accurately in the EMR
- provide reassurance and support.

Why this role
