Registered Nurse (RN) Case Manager, Hospice

The Registered Nurse (RN) Case Manager, Hospice role is onsite in Grandview, WA, serving Benton County and visiting patients in their homes and community settings. Compensation is $40–$55.57 per year, dependent on hospice experience. Heartlinks provides hospice care to patients and families in the community. The nurse assesses patients and families, develops and updates individualized plans of care, provides nursing interventions and education, and coordinates care for an assigned caseload averaging 12–15 patients. The role supports patient and family needs through hospice and palliative nursing care.

What you'll do

• Complete initial, comprehensive, and ongoing patient and family assessments
• develop and revise plans of care
• provide nursing care, prescribed medications and treatments, counseling, and health instructions
• support activities of daily living
• coordinate care for an assigned average caseload of 12–15 patients when assigned as case manager.
