Home Health Registered Nurse (RN) – Torrance

What you'll do

• Provide home health visits
• perform comprehensive assessments and document findings
• obtain and document physician orders
• initiate and regularly reevaluate care plans in consultation with the Clinical Team Manager
• coordinate with physicians and the home care team
• complete recertifications, resumptions of care, transfers, and discharges as requested
• assess need for other team services
• conduct Home Health Aide supervisory visits
• meet regulatory requirements.
