What you'll do

• Assist residents with bathing, dressing, grooming, toileting, and mobility
• measure and record vital signs, weights, and intake and output
• help with meals and monitor nutrition and hydration
• safely reposition and transfer residents
• report changes in condition to the charge nurse
• document care in the electronic health record
• support residents with dementia
• maintain safe rooms and common areas
• follow infection control, fall prevention, and resident rights policies
• communicate with families and visitors
• participate in shift reports, care plan huddles, and training
• support activities and orient new residents.
