Adult Community Support Counselor

Job overview

The Adult Community Support Counselor role is primarily community-based in Derry Village, NH, with work also performed in the office and remotely via telehealth; it pays $24 per hour. Center for Life Management provides outpatient mental health services for adults and elders, including people with persistent mental illness and co-occurring needs. The counselor provides community-based counseling and functional support, helps clients build coping strategies and work toward treatment goals, supports families and natural supports, and coordinates with treatment teams and external providers to maintain continuity of care. Crisis intervention and medication training and support are provided as needed.

What you'll do

• Provide counseling and Functional Support Services in home and community settings
• provide therapeutic behavioral services to develop and reinforce coping strategies
• provide family training and counseling as needed
• provide medication training and support as needed
• provide crisis intervention as needed
• collaborate with treatment team members, natural supports, and external providers
• document services in accordance with agency and regulatory standards
• meet the stated direct-service performance indicator.
