What you'll do

• Complete social histories and psychosocial assessments
• contribute relevant information to the minimum data set and care plans
• maintain required documentation
• support resident self-determination and explain alternatives to refused services
• provide or coordinate therapeutic support for transitions
• educate and support residents and families
• coordinate discharge planning and referrals
• work with the interdisciplinary team to promote resident rights and psychological well-being.
