Interim Temporary Admissions and Transitions Coordinator

The Interim Temporary Admissions and Transitions Coordinator position is hybrid in Minneapolis, MN, with a Monday–Friday, 8:30 a.m.–5 p.m. schedule; pay is $22–$24 per hour. The role supports Nexus Family Healing’s new 13-bed youth crisis and stabilization center in Hennepin County. The coordinator connects youth and families with stabilization services and community supports, and helps plan safe transitions and aftercare. The work contributes to continuity of care by coordinating with families, internal teams, and outside organizations during intake, treatment planning, and discharge.

What you'll do

• Help clients connect with services and identify barriers
• support families and clients
• collaborate on stabilization and discharge plans
• develop treatment plans and provide intake screening and assessments as needed
• coordinate with outside organizations
• maintain records and reports
• facilitate discharge planning meetings
• support aftercare and follow-up
• and protect confidential information.
