Case Manager, Housing Support & Cal-AIM

Job overview

The Case Manager role is onsite in Oakland, CA, with compensation of $26–$28 per hour. Lao Family Community Development provides community, housing, workforce, education, and human services for underserved populations. This role supports people returning from incarceration and participants in the Cal-AIM Initiative by coordinating housing placement, care management, referrals, and follow-up. It helps participants address housing and other clinical and non-clinical needs and work toward stable, permanent housing.

What you'll do

• Conduct intake, eligibility verification, assessments, orientations, workshops, and peer coaching
• develop personal, housing, savings, and Cal-AIM care plans
• connect participants with resources and referrals
• recruit and coordinate with landlords and service providers
• maintain records and compliance documentation
• coordinate with health providers and managed care plans
• track participants and provide follow-up through program exit.
