What you'll do

• Provide health guidance and education
• maintain a caseload of approximately 12–15 clients and conduct regular home visits under the NFP model
• assess needs, support referrals, and maintain client records
• teach and orient nursing staff
• participate in quality assurance and performance improvement
• liaise with community providers
• support program guidelines, staff communication, and program events.
