Care Coordinator, Transitional Aged Youth

The Care Coordinator role is site-based in Crockett, CA. Compensation is $24 per hour. The position supports youth ages 18–24 in Hope Solutions’ transitional housing program. Hope Solutions works to address homelessness and poverty through permanent housing and support services. The coordinator helps residents build independent-living skills, strengthen support networks, and work toward housing stability. The role contributes to a safe, structured, trauma-informed environment and supports youth as they search for and transition into housing.

What you'll do

• Maintain a safe, welcoming, and structured environment
• build trusting relationships with youth
• support daily living activities and individualized service plans
• use nonviolent crisis intervention and collaborative problem-solving
• coach life skills
• assist with housing searches and transitions
• conduct safety rounds and checks
• document observations, interactions, incidents, and services
• help residents navigate community resources
• complete required administrative records and attend meetings.
