What you'll do

• Conduct comprehensive assessments using CANS
• implement Level I, II, or III care coordination services
• develop and implement client-centered Care Plans
• facilitate Child and Family Team meetings
• provide advocacy services including referrals and support
• monitor client engagement
• maintain accurate documentation
• coordinate with community resources and treatment providers.
