Rapid Rehousing Case Manager

Rapid Rehousing Case Manager is an onsite, full-time role in Boston, MA, with a flexible schedule that may include work outside regular business hours. Pay is $26 per hour. The role supports Bridge Over Troubled Waters’ Rapid Rehousing Program for homeless youth and young adults ages 18 to 24. The case manager helps participants move toward stable, permanent housing and self-sufficiency through individualized service planning, case management, life-skills support, and connections to community resources and opportunities.

What you'll do

• Conduct housing needs assessments
• identify barriers and develop individualized service plans
• manage a caseload of 30 participants
• provide emotional support, budgeting and life-skills training, job-search and job-readiness advice
• connect participants with community, healthcare, career, and education resources
• maintain client files, logs, outcome reports, and program data
• participate in program and staff meetings.
