Occupational Therapist (OT) - Home Health

Provide occupational therapy services in accordance with the physician’s plan of care and state practice guidelines. Develop individualized treatment plans and establish functional goals to improve patient independence. Communicate plans of care with physicians and interdisciplinary team members. Teach new skills or retrain patients in activities of daily living impacted by illness or injury.
