What you'll do

• Assess counseling needs and use standardized assessment tools
• determine appropriate level of care
• obtain relevant prior records
• collaborate with clients on treatment plans
• provide planned substance-abuse counseling
• review progress and revise plans as needed
• develop discharge or transfer plans
• coordinate with referral sources and multidisciplinary teams
• document counseling, assessments, referrals, and follow-up
• conduct intakes and orient new clients
• monitor urine specimens as required
• complete discharge paperwork
• respond to guest medication calls as required.
