What you'll do

• Assess patients in their homes and identify changes in condition
• develop, implement, and evaluate nursing care plans
• administer medications, wound care, IV therapy, and other prescribed treatments
• monitor progress and identify potential complications
• educate patients and families
• coordinate with physicians, therapists, caregivers, and other healthcare professionals
• maintain clinical documentation and follow infection-control, patient-safety, and home-care standards.
