Geriatric Physician – Rural Care Facility

The Geriatric Physician is an onsite, full-time role in Rural Retreat, VA; compensation is not specified. The physician will work in a small, early-stage elder-care organization focused on preventative care, health management, and treatment for older adults. The physician leads clinical decisions and coordinates interdisciplinary care to preserve function, reduce avoidable acute care, and align treatment with patients’ goals. The role also helps shape clinical protocols and workflows as the organization grows.

What you'll do

• Complete comprehensive geriatric assessments and care plans addressing function, cognition, medications, goals of care, and social risk
• identify early decline and coordinate interventions
• reconcile and simplify medication regimens
• partner with patients and families on advance care planning
• guide interdisciplinary care
• monitor chronic conditions and functional stability
• review outcomes, close care gaps, and standardize geriatric protocols.

Why this role
