What you'll do

• Collect, validate, analyze, and report clinical quality, operational, population health, and service-line data
• support quality measures, patient outcomes, access, care gaps, chronic disease management, and preventive screenings
• prepare reports, dashboards, scorecards, and presentations
• maintain patient registries and care-gap lists
• collaborate across clinical, operational, compliance, IT, billing, and ancillary teams
• support improvement projects, audits, accreditation, HRSA reporting, payer programs, and grants
• train staff on documentation and reporting
• maintain confidentiality and comply with HIPAA and information security standards.
