What you'll do

• Coordinate clinical discharge planning and patient progression
• collaborate with emergency department and admitting providers on admission and documentation
• complete assessments and progression documentation
• monitor clinical and discharge milestones
• identify and escalate barriers and delays
• participate in interdisciplinary rounds and progression huddles
• support utilization management and clinical documentation integrity
• document assessments, plans, referrals, and patient interactions.
