What you'll do

• Provide clinical support by phone, email, or video and respond to urgent clinical escalations
• triage symptom surveys and reported symptoms
• coordinate care, nutrition support, discharge planning, and referrals
• educate patients about diagnoses and care options
• build trusting patient relationships
• collaborate with clinical staff and health partners
• follow documentation and operational standards.
