What you'll do

• Complete and review initial and renewal assessments
• update individualized plans of care
• review care documentation and address deficiencies
• conduct supervisory visits in clients’ homes
• evaluate skilled nursing and personal care services
• monitor patient condition and communicate changes
• educate caregivers
• participate in audits, chart reviews, quality assurance, and corrective action planning.
