Case Manager

Job overview

The Case Manager role is onsite in Concord, NC, with a Monday–Friday schedule from 8:00 AM to 5:00 PM and 30–40 hours per week. Pay is $23–$25 per hour. Cabarrus Health Alliance works to improve individual and community health through services, education, and collaborative action.

What you'll do

• Provide case management
• conduct intakes and enrollment
• follow client care plans
• track data and prepare monthly reports
• document services in the electronic health record
• accompany clients to meetings and participate in community group meetings to advocate for client needs.

Why this role

Work spans clinical, community, detention, and harm reduction settings. Qualifications may be met through a relevant bachelor’s degree plus two years of related experience, or a high school diploma or equivalent, four years of related experience, and peer support specialist certification; equivalent education or experience is also considered. Lived experience is preferred.
