What you'll do

• Oversee and coordinate the Social Services Department
• complete psychosocial assessments, care plans, and required documentation
• provide input for the MDS process
• advocate for residents’ needs
• build relationships with residents and families
• participate in interdisciplinary care conferences
• assist with discharge planning, transfers, and community resources
• address concerns and support problem-solving
• maintain regulatory compliance.
