Registered Nurse Case Manager – Pediatrics is an onsite, full-time role serving San Diego County and Riverside County, CA, with work hours Monday through Friday, 8:00 a.m.–5:00 p.m. Pay is $45–$61 per hour. The Elizabeth Hospice’s mission is to enhance quality of life for people nearing the end of life and those who grieve. The nurse assesses patients with input from the interdisciplinary team and family, develops and coordinates care plans, and evaluates whether care meets patient and family needs. The role also provides nursing care, patient and family instruction, and supervision of assigned LVN and home aide services.

What you'll do

• Develop written plans of care with the interdisciplinary team
• oversee and evaluate plan implementation
• direct and supervise LVN and home aide services
• communicate changes to the team and primary physician
• document care electronically
• provide health care instruction
• orient and train assigned personnel
• participate in 24/7 patient and family coverage, including on-call.
