What you'll do

• Develop referral partnerships with cardiology, neurology, orthopedic, and urology providers
• educate physicians and office staff on home health utilization
• identify patients who may benefit from skilled home health
• coordinate transitions from hospitals, surgery centers, clinics, or physician offices to home
• manage referrals through admission
• provide education about Medicare home health eligibility
• work with clinical managers, intake, and operations
• maintain communication with referral partners and leadership
• achieve monthly referral and growth goals.

Why this role
